Member Data Forms

New Members should start with the Family Information Form.  This provides the data used to populate your family information in our database and church directory.  The Sign-Up to Serve form is used to volunteer for the areas where you are willing to serve.  New members may be ready to fill this form out immediately, or may choose to wait a few weeks.  The Change of Information form is used to update things like address, phone, and email.
